Change of Student Information

Update your official college records by completing a Change of Student Information form and submitting it with the required documents to the Enrollment Services Center.

Name

Documentation (e.g. marriage license, court order) is required for a change of name.

Social Security Number (used only on the Application for Admission)

An original Social Security Card is required for a correction to a social security number.

Street Address
